How to make an exit for babies?

My wife is going to deliver our baby on July. Somebody said, the passport of our baby can attach to my wife's passport. In case our baby is living Qatar with our relative only not with my wife, is anybody have ideas how to make an exit for our baby or what documents that our relative should take during the travel or exit of country?
Please help

Babies should be feed on mothers (breast feeding) for the first six months at least.

Once baby born get birth certificate from HMC register in Phillipine embassy, get seprate passport for baby, RP formalities. To exit no visa required.
