Increasing number of Medication Errors at HMC
Dear All,
Medication Error at HMC parmacy is increasing day by day. Someone here might had suffered or heard about it. The error is Parmacy swaps the medication bag between two patient i.e giving medication bag of a patient to another patient. For sure they are not doing this puposely. Its a mistake occuring due to lack of staff or more no. of patients waiting at window for medication or may be there are two pt. at window and the medication bag swaps there or whatever but HMC is trying take preventive steps as much as possible to recover from this error.
This happened with me twice and could happen to any of you here. Might possible that a medication of high dose or wrong concentration is given to you or visa versa. Wht would happen if you take awaye medication of a critically ill pt. and visa-versa???
Wht you have to do is a simple step; whenver you are collecting medications for any of HMC facility pharmacy just open the bag (its a single knot) & plz plz plz do check whether the medication box is having your HC no and name sticker or not.

when dealing health issues, it is supposed to be Zero error.
For customers buying at any pharmacy, just double check your prescription to the medications gave to you in the pharmacy.

Happen Twice to you,its really strange, because they pharmacist are very cautious before giving medicine, even they ask your name before issuing the medication, that is the normal procedure, but may there is another patient same name for you there, otherwise its very rare chance

As far as I know, medical ethics includes the accuracy of providing medication on the pharmacological aspect of medicine, because we are dealing with life here. And in worst case scenario, negligence on the part of the pharmacist would lead to revocation of professional license.
Mistakes happen, but not all the time.... so next time be careful.
As a patient, it is our right to ask and check all the procedures & medications that will be given to us.
thanks for the info, I never take any medicine until I have read up on it - as I am very critical about the taking of any medication in general.....

well tallg and chuck i m at one of hmc's hosp. from 7-3 and also the one who recieves and maintains this data. medication errors are of many kinds reported by/thru/against pharmacy. i mentioned this particular one coz atlest not hmc but the pt. itslf can take preventive measures to over come. after all this error will cause nuthin to pharmcy but to the patient.
also please note its a patient right to report such errors to public relation department which u will find at all hmc hospitals.
Kimd - this idea that people barge in is not new to Qatar and not specific to pharmacies - see how other drivers are here. I would also like to ask Sadaf how exactly they know this is an increasing problem? Hopefully u're not at the HMC every day. DO you know of many other people who have suffered the same issue as u?
sadaf - Good advice to check the medication is yours, but I'm intrigued as to where you got your information from. You say that 'Medication Error at HMC parmacy is increasing day by day'. Do you have a source? Some figures?
I visit HMC regularly and have only once,in 15 years of visits, been given the wrong medication at the pharmacy- it was the right medicine but the wrong version.
However I do get very frustrated by the length of time it takes to receive prescriptions- sometimes up to 40 minutes.My biggest gripe is that people don't know how to queue, you could be standing waiting your turn to be served when someone just comes and shoves in front of you waving their appointment cards/prescriptions at the HMC worker. It's like you're invisible.
