how do i get an health card??? My Husband does not hav any health card.... what are the formalities and wer do i get the forms???? kindly advice..tanx in advance
you can go to nearest primary health center or hamad hospital requirement is your passport copy with valid resident permit 2 photographs and fill the form and 100QR is a fee for each card
Bring your resident permit (pataka), picture, electricity/water bill to the nearest health center in your place and you will be given an application form to fill up there. Don't forget the copy of your water/electricity bill for this will confirm your address. Health centers have jurisdictions as well with regards to your location/address. visit nearest health center with valid RP (ID)with 100qr and remember photograph I got it from the one near Jeddah tower.
If you have a valid RP, you can visit your nearest Health Centre and fill up the form, attach passport and visa copy and submit.
Or your husband company HR dept can assist him if he approach them.
