Which documents are required to apply for family visa (For Indian).

I need to know which documents are EXACTLY required (other than the below) to apply for the family visa.

Do I need get a letter from the company saying they had no objection if I get my family here and my job profile on the letter?

Do I need to get my wife's name written on my passport from Indian Embasy?

Do I need to get the passport copy of my family members attested from the Indian Embasy?

Is it necessary to get the contract copy stamped and attested by labour ministry or a letter from company serve the purpose?

My company was giving me salary in cash for the last 10 months just last month I have a bank account here in Qatar, do I need a seperate letter from my company stating this as my bank statement is only one month old?

I think if anyone submit all the correct documents there will not be any delays or cancellation from the ministry, thus please let me if anyone have any suggestions.
