3. Parent/Guardian Consent letter (if minor =24 yr. old below)
4. 1x1 pictures (4pcs)
5. NOC from you company (both of you)
6. Application fee of 500 from Phil. Embassy
Note: All certificate must be DFA Authenticated (Red Ribbon)
