Report Birth Abroad

The Consular Report of Birth Abroad is the official birth record for children born in Qatar to a U.S. citizen parent(s). Along with your U.S. passport, a Consular Report of Birth Abroad is an important document that shows how an American acquired U. S. citizenship.

The following documents are ordinarily required to issue a Passport and/or a Consular Report of Birth Abroad to a child born in Qatar to a U. S. citizen parent or parents:

Birth certificate issued by the Qatari authorities with English translation. The original Qatari birth certificate is issued by the Ministry of Public Health, Preventive Health Department on the basis of a "Notification of Live Birth" form issued by the hospital in which the baby was born.

The marriage certificate (any marriage certificate not in English should be translated to English. Non-U.S. marriage certificates are acceptable). If the parents are not married, please ask a consular officer about special requirements.

Evidence of the termination of previous marriages (with translation if not in English), if either of the parents were previously married. Normally, such evidence consists of a divorce order, annulment decree, or death certificate.

Evidence of U.S. citizenship of both parents. Such evidence normally consists of a U.S. passport, naturalization certificate or a certificate of birth from the United States. If only one parent is a U.S. citizen, the U.S. citizen parent must complete the Affidavit of Parentage and Physical Presence and provide evidence of at least five years physical presence in the United States, two years of which must have occurred after the age of fourteen. Evidence of the U.S. citizen parent's physical presence in the United States could include old passports, educational records, rent receipts, etc. Please complete the following Affidavit of Parentage and Physical Presence form.

Two passport photographs of the baby. Photographs should be 2" by 2" with a white background and full face. The baby should be clearly identifiable from the photograph. (Photograph Specifications).

Fee: The US Embassy charges $65 US for a Consular Report of Birth Abroad. The fees can be paid is USD or QR. Other Fees may also be applied.

Please complete DS-11, DS-2029, SS-F-FS forms.

If the child is over the age of eighteen, a Consular Report of Birth Abroad will not be issued but a passport may be issued once the identity of the child has been ascertained and if the following documentary requirements have been fulfilled.

All documents must be submitted in original or certified copy, and 2 photocopies. Both parents and the baby must be present and the documents will be reviewed by the Consular Officer.
