Which Health Center for applying Health Card -family (AZIZIA)

Dears,
I am trying to find the health center nearest (Azizia/Hyatt plaza/Zoo) to apply health card -(for me & wife). I went to Abu Barker and now it is only for Qatari. My friend told Rayan is for family with children. I am confused where to find the health center. Please advise, any body recently applied.

Finally I found the place- Abu Nakhla. I submitted both documents for my wife and me. Sametime, they gave me numbers to collect health card. I went to hamad Hospital and collected.

For bachelor, go to al wafeedin health center near b-ring road al munthaza.
