Procedure to apply Hamad health card for Family
Hi All,
I want to know the procedure for applying health card for family.
I already have Hamad card for myself issued at Health center (21) near Apollo clinic signal, Al Mansoura.
I also got the Transfer certificate for moving my card from Health center (21) to Mesaimeer health center.
Please inform the procedure for applying health card for my wife and child:
1) Is there any application form to be filled? If so, from where I can get the forms?
2) Documents to be attached for applying health card for family
3) Timings at Mesaimeer center for submitting application.
Can we apply on all 5 days in a week?
4) Does my wife and child need to come Mesaimeer center to apply for health card?
5) Fees for applying health card for myself (transfer), wife and child?
6) After submitting the application form, by when will the cards be issued and mode of delivery?
P.S: Wife and child are having Residence Permit and Qatar IDs, under Husband sponsorship.

1. Yes, there is a application form to be filled. But from reception they will do it from the particular PHCC
2. You need your original passport, your and your spouse Q.I.D Copy, 1 Passport Size Photo and most importantly your Electricity Bill, if bill is not in your name, they may ask you the House Rent Agreement as well, but this is purely to verify you are in the particular PHCC area.
3. 7.30 AM to 11.30 AM.
4. Not Required.
Timings starts from 07.00 AM but prefer to go there around 8AM.
Cash is needed (not there as they do not have card printing facility). After filling the form there with PHC numbers you have to either visit main Hamad or Primary health centre (nearest one is Omar bin Khitab in new salata area)
