Not sure which countries have 200Mbps, but back in the U.S. Verizon FIOS internet is considered the fastest out there. FIOS is a new service that is being offered, that only started couple of years ago, and only very recently they started having the 50Mbps connection. By very recently I mean while I was there in 2009 they only had the 25 one.
As for streaming not sure why you cant since internet here is up to 8Mbps and that is more than enough. Even in the U.S., unless you have something similar to verizon's FIOS you wont have much higher speeds than the ones offered here. http://www22.verizon.com/residential/fiosinternet#plans
