i believe that you can only have a visit visa for 3 months and then you have to leave the country to dubai or bahrain for the day and then get another 3 months when you re-enter the country, i have had to do this before and only when its a medical reason that you cant fly that they will extend beyond the 3 months, good luck!

yes you can extend family visa to total of 6 months. Do medical checkup then apply for extension with medical result
