hi QL, i am working in Qatar, wish to take passport to my child, she is in india, 2 years old. which form need for getting affidavit? pls. help.

Visit the below link and download form C1 (Sworn affidavit), get it attested from the Embassy and send it to India.

They have all the forms and requirements listed there. If parents have all necessary documents to prove their residence, the whole affair would be very easy.
