Hi, I did this myself recently so it's quite fresh in my mind. First you need to go to an opticians and have an eye test specifying that it is for a driving licence. If you pass your eye test they will get you to fill in a form which then needs to be taken to the driving licence offices. You will need either 1 or 2 passport photos (can't remember if it's 1 or 2) when you have your eye test, one will be fixed to your eye test form and stamped.
Next you take this form along with some more passport photos, your UK driving licence, your RP, passport, shoe size, underwear colour etc. to the driving centre, along with the fee QR 250. They will bounce you around some varying levels of police seniority to gain the relevant signatures inside the department, and then you take the completed document to a desk where a not very cheerful operator will debit your card (payment must be by card) or QR 250, and give you your brand new driving licence there and then.
