just visit any qtel outlet... fill up the form, tick the replace card option and attach ur valid id copy or passport copy. pay 50 riyals and get the new sim card with the same number.
u have to go to any qtel office and request for that, that is as far as lost sim card for postpaid subscriber and also for having the same prepaid number reactivated after it expired but you have to pay QR200, if im not mistaken, for that...

just go to Qtel and complain. you will get your new sim card with same number within 10 minutes.
