Lost - Child Vaccination Card - Need advice

Dear All,
I lost my daughters vaccination Card and i dont remember where i had kept it /or forgot to collect it from PHC when i had visted them few months ago.
Do you know how to get a new card and enter all the vaccination records in that again? i have the HMC and PHC number with mme
First i got it from NHA ( My daughter born in India) showing hers india vaccination record. then i visted Rayyan PHC for vaccination. Now she is 2 years 7 months old and completed all the Vaccinations except one is scheduled for 4 years og age.
I think i need the this card when i have to join/admit her to Nursery/KG.
Please help me with steps which i need to take in-order to get a new card.

I lost the vaccination card of my child and got a new one at our health center - everything went smoothly -it was free of charge / the NHA building near LULU center does not exist anymore ...
Y I dont know only half of it came. so its a continuation.Contd: U need to pay them QR100. They will ask for the contact details. They will ask U to wait for sometime to issue the Medical card ... from there U can go to Messaimeer Health Centre and get a new Immunization card. Please go back to the health centre where u put Vaccination, and get the detailed filled up in that new card. ALL YOU NEED TO DO IS " GO EARLY AS POSSIBLE AS U CAN" SAY 7AM., before U do all these. ALL THE BEST. INSHA ALLAH. U will get it.
Hey.. I got my Kid's New Vaccination Card... All You need to do is:Go to Health center where U put Vaccination for the baby. I went to Airport Health Centre. There is a lady who is so kind and she was very helpful.If U have Medical card for ur baby, then U can go directly to Messaimeer Health centre where U get that Green colur Immunization card, once U show the medical card and tell them U need the new card.If U dont have the Medical card, apply for one, by giving them a passport size photo and the baby's Health card number which starts with HC------- or Birth Reg number, and a passport photocopy.U need to pay them QR100. They will ask for the contact details. They will ask U to wait for sometime to issue the Medical card ... from there U can go to Messaimeer Health Centre and get a new Immunization card. Even I am in same situation. When I went and asked whether I can re-apply or I can apply medical card with the number, they said that they need that Green colour Vaccination card.
I am still waiting to see somebody out there can give me some advise.
