KOLKATA: If you thought you'd never be off those insulin-filled injections, here's some good news. Type-II diabetes - or diabetes mellitus - can now be cured by laparoscopic surgery. Here in Kolkata.

On February 18 this year, the first such surgery in India was conducted at Hyderabad's Kirloskar Hospital. A team of doctors - Surendra Ugale, Neeraj Gupta and Vishwas Naik - operated on a 56-year-old woman. The same team is now ready to offer this treatment at Belle Vue Clinic here.

Ugale, who has been flying to the city once a week for pre-surgery consultations, says, "People are overjoyed when we tell them diabetes can be controlled and cured by surgery. So, we have started giving them demonstrations of the procedure." Patients will be selected only after conducting C-Peptide tests on them to ensure that their pancreatic functioning is normal.

Ugale has been trained by the world pioneer of this procedure (ileal transposition surgery or small intestinal switch), Dr Aureo at Goiania, Brazil.

He says, "We can begin the operation in another three months. The patients will be investigated, selected and all their doubts cleared before they undergo such a major procedure." His target group: between 20 to 65 years - those who have been on oral drugs or insulin for three to 12 years.

The surgery costs patients in Hyderabad around Rs 3.5 lakh. In Kolkata, the expense will be Rs 5 lakh initially, to be reduced once a self-sufficient team takes charge here.

The operation is done by the bloodless key-hole surgery, using hi-tech stapling devices and blood vessel self-sealing devices. The devices manoeuvre the lower part of the small intestine (ilium) causing a surge in insulin hormone that controls blood sugar. The patient must stay in the hospital for eight to 10 days for observation - especially to treat dropping blood sugar levels.

The surgery can arrest body organ deterioration and thus avoid future diabetic complications. Once the operation is over, the insulin hormone goes up as soon as the patient takes food, thus controlling post-prandial sugar. This way it's better than tablets or insulin as it reduces the chances of hypoglycaemia (sudden lowering of sugar level).

Echoes Ugale, "Once the surgery is performed,. the sensitivity of target cells increases so that fasting sugar levels dependent on liver are better controlled. Also, the progressive destruction of insulin-secreting B-cells in pancreas is arrested. This way, the surgery controls sugar in the most physiologically natural way enhancing the effect of body insulin."
