Anyone know where I can get the following Car Shipment Letters / Documents?

I am trying to ship my car home that brought here from the U.S. I was told that I need "Certificate of origin from Chamber of Commerce" and I have no idea where I am supposed to go and get this, or even begin the process. Any information would be helpful. I was also told that I need "Letter to the customs manager asking for the permission to export". I have been to 3 typing services and they all look at me like I am from another planet. If someone has an example I could use to show these guys that would be great. Any help from you all will be greatly appreciated as well.
