You will only need your old Qatar ID Card. Just to be on the safe side, do carry your passport with valid Qatar RP.
Once you get the Smart Card, there is a kiosk at the entrace of Gate 1 where you need to activate the Smart Card for e-Gate and e-Gov Services. It wouldn't work without this activation.
For me they charged me Qr500/- for the smart ID card, which I thought was too much for something that was not mandatory. I couldn't apply via the Hukoomi website, coz it needed an Establishment ID parameter, which my company didn't know, hence couldn't provide.
So I went directly to the Immigration Office at Gharaffa.
