You will only need your old Qatar ID Card. Just to be on the safe side, do carry your passport with valid Qatar RP.

Once you get the Smart Card, there is a kiosk at the entrace of Gate 1 where you need to activate the Smart Card for e-Gate and e-Gov Services. It wouldn't work without this activation.

Hi Eric,

What are the documents needed to get the Smart ID card.

For me they charged me Qr500/- for the smart ID card, which I thought was too much for something that was not mandatory. The typist told me that it might cost only Qr150, but it turned out to be way more. Not sure if I was overcharged by the Qatari at the counter who was angry since i did not understand his arabic

Next, take these signed forms to Gate 1, where you have to go to the retinal scan area. Here the machine will take your retina scan, take an electric finger print of both index fingers, ask you to do a electronic signature and also take a photo (I looked sleepy and was not prepared for a photo :(  , so be prepared for the photo), this photo will appear in your ID card.

Next go to the reception and get a token for the Smart ID counter. At the counter they will charge you for the ID card and ask you to wait till your new ID card is ready. That's it!!

Thanks for the reply,

I couldn't apply via the Hukoomi website, coz it needed an Establishment ID parameter, which my company didn't know, hence couldn't provide.

So I went directly to the Immigration Office at Gharaffa.

First, go the typing center and request for the form for Smart ID card. They will create two forms in arabic (They might charge you Qr10 for this). If you want e-gate facility, don't forget to ask them to include that in the form.
