if u want to reniew then u dont need to submit any kind of application for health card,
just take ur Passport copy with valid visa + passport size photograph along with fees & go to nearest health centre , they will make u health card in 10 minutes

if u want to have a new 1 then u must first register ur primary health number at nearest health centre & follow the same above process
