Go to the nearest Health Center where you stay. Go to the reception, they will give you a form to fill. They will check to the system and get the HMC number for you. Usually you have to come back one day after that day. After you get HMC number, then you have to come to Health Card section, submit your form. Pay QR 100 each member of your family, get the recite.
