For QID card, go to any police station. They will issue you a letter. Take this to immigration to get new ID card made.

For Driving license, go to traffic police department. They will issue you also a letter. With this letter you can continue driving as normal, and use it as a license. You have to use this for about 2 weeks, after which you can return to traffic police department to make new license.

For health card, just go to health centre - to the health card issuing counter, and they will issue you new card.
