b. go to administration side of women hospital HMC 3rd floor, the new born reg, centre.
c. fill the form, a very simple, easy to understand.
d. attach ur PP, RP, Wife PP & RP, HMC Cards & marriage certificate.
e. select the number of copies, normally 3 copies are necessary to possess.
3. if the baby born in other hospital;
a. take vaccination card & birth report issued by the hospital.
follow the b. c. d. & e. but do get the HMC birth entry number from the women hospital Ground Floor. normally the mens' entry prohibited there, but it is well managed by a helper assisting in this business.
