4,Application form you can get from any near by typing centre it costs only 5 riyal.
they will give you an entry visa with 1 month validity and you can extend up to 5 more months.(5+1).You have to pay 200 riyal for each month.
The medical can be performed within one month,if you want to stay more than one month.
Once your exit from Qatar the visa will expire automatically and have to wait 3 months in home country for getting next visit visa.
