How can a US citizen get a FS-240 application for duplicate birth certificates?

I need to submit a copy of my children's birth certificates to the Ministry of Employment in order to complete their Residency Visas. She has a US passport and all we need is a duplicate CRBA.

The FS-240 which is the application form for this situation is no longer beng printed locally (at the US Embassies)! Where can we get this form??

Additionally, the service charge is $50 which needs to be paid by money order. Does anyone know where I can purchase one from?
