what are the requirements for visa application baby born in qatar?

I just want to ask what are the documents i need to
prepare for the application for visa for my baby born
here in qatar...she was born on July 4 and i just got
her birth certificate last July 20. Her passport will
be released on August 28 because it takes 1 month for
Philippine passport to process...

Here is the documents required to obtain visa for new born baby:
1.) Application which you can get in the portabin outside the immigration.
2.) Colored passport pic any background will do except white.
3.) Original Passport of your baby.
4.) Passport copy, ID & RP and Marriage certificate copy of HUSBAND & WIFE.
5.) Birth certificate copy. Both the white & yellow.
If you got the approval immediately then be thankful because once I've applied for my baby and they did not give me the approval unless I'll transfer my wife sponsorship.
NOTE: Make sure you process this before 2 months starting from the birth of your baby or else you'll be fine 10QR per day.

That's it-as far as i remember when i'd processed visa for my baby born in qatar long back...Better ask your company PRO if i've missed out any other details..
