As per my knowledge istimara is suppose to be renewed every year. Check details in your existing Istimara Date of Valid Till.
First you need to go through inspection, you can do this in the industrial area, or which i recommend next to abou hamour petrol station (75QAR fees)
Once your car will pass, you will have to renew your insurance then you can register your car again (i recommend to go to Mourour Rayan next to aspire zone, they have there people who will do this second process)
